Diagnostics and laboratory testing is two structurally different businesses wearing one label: a $110-130B global in-vitro diagnostics products market (2025E, Grand View Research), and a substantially larger clinical laboratory services market for which no single consolidated figure is defensible given inconsistent scope across sources. Growth is accelerating — 5-7% forward against 4-5% in the post-COVID normalization years — on molecular diagnostics and companion diagnostics tied to targeted therapies. The two sides should be read separately; they share a supply chain and little else.

Market structure

The IVD products side is concentrated. The top 5 majors — Roche, Abbott, Danaher, Siemens Healthineers and Thermo Fisher — hold an estimated 50-55% of global share. Lab services are the opposite: fragmented everywhere outside a US reference-testing duopoly held by LabCorp and Quest. Revenue concentrates in the US and EU; manufacturing in the US, Germany and Switzerland.

Reagent and instrument manufacturers sit upstream; hospitals, physician offices and payers downstream. The economics follow the classic instrument-plus-consumable model — an installed base of placed instruments drives recurring reagent and consumable revenue at 55-65% gross margins. But realized price and volume are set outside the building: CPT and CLFS codes and payer contracts are the primary driver, so coding decisions no single lab controls determine what a test actually earns. A concentrated products franchise and a fragmented services layer face entirely different competitive pressures, which is why the single label misleads.

How AI is reshaping this segment

The clearest labor-substitution case in the sector sits here. Agentic AI is automating pathology and image review and result interpretation, compressing pathologist and lab-technician labor cost directly. Algorithms now match specialist accuracy on defined interpretation tasks, and that undercuts the manual-interpretation moat that has historically justified premium pricing on complex pathology work. Digital pathology and AI-assisted imaging are already underway — none of this is a future-state prediction.

The expansion map has two arrows pointing in different directions. Companion-diagnostics partnerships add pharma partnership revenue tied to targeted-therapy launches — a genuinely growing category as more drugs require a companion test for approval. Reference-lab consolidation is about defense: scale and reimbursement leverage in a fragmented services market where negotiating power with payers is a function of size. The broader adjacency into genomics and precision medicine — digital pathology in particular — is where diagnostics and the life-sciences-tools segments increasingly overlap, and where much of the segment's forward growth is concentrated.
